Your device and operating system version, such as iPhone 14 with iOS 18 or Pixel 9 with Android 16.
What you were doing and what happened. A screenshot is ideal.

Reset your password: tap Forgot password? on the email sign-in screen and request a new six-digit code.
A lab or visit was read incorrectly: review and correct structured information before saving. You can also edit saved information in the app.
Delete your account and data: open Settings, Account, Continue to delete account, or use our web deletion request if you no longer have the app. This action cannot be undone. See the Privacy Policy for backup retention.
